Community Nursing Services

(Now accepting Plan Managed and Self-Managed Community Nursing referrals)

CASS Community Nursing Services are nursing supports provided to NDIS participants to help manage their health needs while living in the community. These services are designed to promote independence, wellbeing, and safety outside of hospital or clinical settings.

Eligibility

  • People with disability
  • Individuals with relevant NDIS funding or funding from any other approved source

Service Fee

The service fee is set in line with government recommendations.

Services

  • Health Monitoring: Regular checks of vital signs and overall health status.
  • Medication Management: Administering medications, injections, and monitoring adherence.
  • Wound and Pressure Care: Dressing changes, infection management, and skin integrity support.
  • Support for Chronic Conditions: Assisting participants with conditions such as diabetes, respiratory illnesses, or heart disease.
  • Rehabilitation Support: Post-hospital or post-surgery care, and assistance with therapy programs.
  • Health Education: Teaching participants and carers about managing health conditions, self-care, and healthy lifestyle choices.
  • Collaboration: Working with doctors, allied health professionals, and support staff to coordinate care.
